Our NDIS Services

Victorian Home and Community Care provides a range of NDIS services with a strong focus on capacity building. VHACC is committed to working with participants who require a service that recognises the importance of social, family and community connections and working alongside professionals from multiple service systems.

We can assemble a team of support workers and train them to meet your specific needs by drawing on the mix of expertise across our leadership team. To date we have worked effectively with participants who experience:

  • Mental health and psycho-social disability

  • Physical impairments creating falls risk and immobilisation

  • Acquired Brain Injury

  • Visual impairment and vision loss

  • Depression and mental health impacts arising from the acquisition of disability

  • Substance use in addition to or arising from a disability

  • Changing needs and disrupted lifestyle due to periods of hospitalisation and acute episodes of a disabling condition

SUPPORT COORDINATION

  • Assistance with implementation and understanding your NDIS plan

  • Supporting you to link your my gov account to the NDIS to access your NDIS plan

  • Supporting you to find appropriate service providers and assistance negotiating service agreements

  • Assistance with finding appropriate accommodation that meets your needs

  • Building your capacity to manage your own plan in the future

Cleaning Windows

DOMESTIC ASSISTANCE

  • Bed making including changing linen

  • General household cleaning and tidying

  • Preparing meals

  • Laundry assistance

COMMUNITY PARTICIPATION AND TRAVEL/TRANSPORT

  • Assistance with shopping and outings

  • Assistance to attend medical appointments

  • Helping you to meet friends and attend social activities in community recreation, leisure and sport etc.
